You'll meet people where they are—in their homes, shelters, hospitals, and communities—and help connect them to the care and resources they need to move forward.If you have experience in case management, behavioral health, social services, discharge planning, care coordination, or community outreach, we want to hear from you!What You'll DoYou'll serve as a trusted partner to clients during some of the most important transitions in their lives.Your day-to-day work may include:Helping clients transition safely from hospitals, psychiatric facilities, detox programs, and rehabilitation settings back into the community.Building relationships with clients through face-to-face outreach and community visits.Developing individualized care plans based on each client's medical, behavioral health, and social needs.Helping clients schedule and attend medical and behavioral health appointments.Connecting clients to housing, benefits, transportation, food, treatment, and other community resources.Helping clients navigate complicated healthcare and social service systems.Staying connected with providers, hospitals, discharge planners, family/support systems, and community organizations.Identifying barriers to care and finding practical solutions.Advocating for clients and helping them work toward greater stability, independence, and wellness.Maintaining timely and accurate documentation and care coordination records.Who We're Looking ForYou don't have to have the exact title of "Care Manager" on your resume to be successful in this position.We are interested in candidates with experience in:Case management, Behavioral health, Social work, Community outreach, Human services, Hospital or discharge planning, Housing services, Substance use services, Care coordination or Other direct-service positions working with vulnerable populationsRequired:Bachelor's degree in Social Work, Psychology, Human Services, Nursing, Public Health, or a related field.At least 2 years of experience working with individuals with serious mental illness, co-occurring disorders, chronic health conditions, or other complex needs.Strong communication, organization, and problem-solving skills.Ability to build trusting relationships with clients and community partners.Ability and willingness to work in the community and conduct regular face-to-face visits.Reliable transportation and ability to travel to client and community locations.Preferred:Experience working with Medicaid, housing programs, benefits, behavioral health providers, hospitals, or community-based organizations.Bilingual/multilingual skills.Master's degreeThis Is a Community-Based PositionIf you enjoy getting out into the community and meeting clients where they are, this may be the right position for you.You may spend your day visiting clients at:Their homes Hospitals or rehabilitation facilities Community agencies Shelters Other locations throughout the communityEvery day can look different—and that's part of what makes this work meaningful.Why Join Our Team?You won't be doing this work alone.At New Horizon Counseling Center, we believe care managers deserve the same support they provide to their clients.You'll receive:Supportive supervision and mentorshipA collaborative team environmentOngoing training and professional developmentOpportunities for career advancementMeaningful work that directly impacts people's livesCompetitive salaryComprehensive benefitsWhy This Work MattersSometimes one person making the right phone call, showing up for a client, helping with an appointment, or connecting someone to housing can change the direction of that person's life.As an HH+ Care Manager, you can be that person.If you are compassionate, dependable, resourceful, and ready to make a difference in the community, we want to meet you.Apply TodayNew to Health Home Plus?
