Hospice RN Case Manager | Compassionate Home Care

Care Initiatives, Inc. is looking for a compassionate RN Case Manager to join Care Initiatives Hospice in Cedar Rapids. In this role, you'll provide exceptional, personalized care to patients during their final stages of life, working closely with families to meet their unique needs. Your responsibilities will include conducting assessments, developing care plans, and ensuring high-quality documentation. Candidates should have CPR certification, at least two years experience, and the drive to make a difference in patients' lives.
