LVN Nurse

Job Summary:

The Licensed Vocational Nurse (LVN) is responsible for assessing, planning, and implementing nursing care for residents within a skilled nursing setting under the supervision of a Registered Nurse and Director of Nursing. This role includes medication administration following strict safety protocols, supervising CNAs and supportive staff, participating in resident and family education, and advocating for residents' rights. The LVN ensures continuity of care, documents patient conditions accurately, and collaborates with the interdisciplinary care team to meet residents' physical and psychosocial needs. Responsibilities:

• Assess residents' conditions and nursing needs; prescribe appropriate nursing actions.
• Administer medications following the eight medication rights to reduce errors.
• Perform and interpret waived point-of-care testing according to policies.
• Document changes in residents' conditions and nursing care on progress notes and care plans.
• Notify physicians of changes in condition and follow through on orders promptly.
• Provide thorough shift reports and participate in walking rounds.
• Implement individualized plans of care based on interdisciplinary team goals.
• Educate residents and families regarding pain management and other care aspects.
• Supervise CNAs and supportive staff to ensure quality care and adherence to infection control procedures.
• Serve as a resident advocate, protecting residents' rights and dignity.
• Participate in orientation and performance evaluations of new personnel as needed.
• Maintain calm and effective leadership during emergencies or stressful situations.
• Communicate respectfully and effectively with residents, families, and healthcare team members.

Required Skills & Certifications:

• Graduate of an accredited LVN program.
• Current California Licensed Vocational Nurse (LVN) license.
• Current Basic Life Support (BLS) certification.
• Ability to read and communicate effectively in English.
• Basic computer knowledge.

Preferred Skills & Certifications:

• Experience with Electronic Health Records (EHR).
• Additional language proficiency.
• Previous experience in skilled nursing or similar healthcare settings.

Special Considerations:

• Supervisory role over CNAs and supportive staff within a skilled nursing facility.
• Must follow strict infection control procedures.
• Ability to remain calm and provide leadership in emergencies and stressful situations.
• Focus on resident advocacy and respect for psychosocial, cultural, ethnic, and spiritual needs.

Scheduling:

• Work schedule and shifts are determined by facility needs within a skilled nursing environment.
• May require flexibility to provide continuity of care across shifts.
