The Nurse Practitioner utilizes advanced education and clinical skills to enhance patient outcomes. This role involves addressing a variety of issues that range from moderate to significant complexity.
Our Integrated Home Care Program is an innovative initiative aimed at revolutionizing healthcare for high-risk patients in their own homes. We strive to minimize unnecessary hospital stays and readmissions by offering thorough, patient-centered care to those with medical complexities and functional challenges. Our dedicated team provides proactive, coordinated care right where our patients need it most.
Key Responsibilities:- Conduct thorough in-home assessments and evaluations.
- Utilize both in-person visits and telemedicine to provide care.
- Create and implement individualized treatment plans.
- Address acute clinical issues within the home environment.
- Ensure accurate and detailed documentation.
- Engage in medical team meetings.
You will report directly to the Chief Medical Officer in San Antonio, Texas, and will be required to visit patients' homes in the San Antonio area.
As part of the hiring process, a TB screening test will be necessary.
Required Qualifications:- Must be a Nurse Practitioner or Physician Assistant with over one year of experience in Home Health or a related field.
- Fluency in both Spanish and English.
- Language Proficiency Testing: Any team member who communicates with a patient in a language other than English must complete a language proficiency assessment provided by an external vendor to ensure competency. Applicants will need to take the Interagency Language Rating (ILR) test as mandated by the Federal Government.

Compensation Range:The salary range provided below represents a good faith estimate of the starting base pay for full-time (40 hours per week) employment at the time of posting. Actual pay may vary based on geographic location and individual qualifications, including skills, experience, education, and certifications.
$123,800 - $170,300 per year
This position is eligible for a bonus incentive plan, which is contingent on both company and individual performance.
