As a payor-agnostic subsidiary, we prioritize a team-based care model where physicians lead a multidisciplinary team to support the physical, emotional, and social well-being of our patients.

Position Summary

The Primary Care Physician (PCP) serves as a key leader within our collaborative care environment. We emphasize quality care for our patients, supported by a diverse team that includes doctors, advanced practice professionals, pharmacists, care coach nurses, medical assistants, behavioral health specialists, quality coders, and referral coordinators. This comprehensive approach enables us to deliver an exceptional experience for seniors, ensuring higher quality care and improved outcomes through a concierge model, coordinated services, and strong community ties. Our structure allows PCPs to manage fewer patients, dedicating more time to each individual.

Key Responsibilities:
  • Assess and treat patients in alignment with established care standards.
  • Monitor patient care quality and adherence through data analysis and chart reviews.
  • Facilitate coordination of patient services, including specialty referrals, hospital and skilled nursing facility coordination, durable medical equipment, and home health care.
  • Actively engage with the care team, providing medical expertise during daily meetings.
  • Support the regional medical director and center administrator in fostering a collaborative practice environment through professionalism and approachability.
  • Ensure timely completion of medical records, collaborating with quality coders to enhance coding accuracy.
  • Adhere to policies and protocols set by clinical leadership.
  • Meet with the regional medical director to discuss care quality, outcome data, and policy or record issues.
  • Engage in potential growth initiatives for new or existing services within the center.
  • Participate in the local primary care "on-call" program as needed.
  • Maintain compliance with licensing, certification, and accreditation requirements.
  • Devote 100% of your time to direct patient care, including patient interactions and necessary administrative tasks related to care.
Required Qualifications:
  • Possess a current and unrestricted medical license or be willing to obtain one in the state of practice; must be eligible and willing to secure licenses in other states as needed.
  • Graduate from an accredited MD or DO program.
  • Strong verbal and written communication abilities.
  • Exhibit exceptional interpersonal skills in interactions with colleagues and patients.
  • Fully committed to the "Integrated team-based care" model.
  • Willingness to learn and adapt to a value-based care environment.
  • Outstanding patient service skills.
  • Basic computer proficiency, including email and electronic medical records (EMR).
  • This position involves direct patient interaction and requires participation in our Tuberculosis (TB) screening program.
Preferred Qualifications:
  • Board certification or eligibility for certification (ABMS or AOA) in Family Medicine, Internal Medicine, or Geriatric Medicine is preferred.
  • Active and unrestricted DEA license.
  • Medicare Provider Number.
  • Two to five years of relevant experience is preferred.
  • Experience managing a Medicare Advantage patient panel with knowledge of best practices in a coordinated care setting.
  • Familiarity with Medicare guidelines and coverage.
  • Understanding of HEDIS quality indicators.
