Occupational Therapist (OT) - Home HealthOccupational Therapist (OT) – Home Health Location: Indianapolis, IN Schedule: Flexible / Per Diem / Full-Time Options AvailableAbout the Role: We are currently seeking a compassionate and motivated Occupational Therapist (OT) to join our home health team. In this role, you'll have the opportunity to make a meaningful impact in the lives of patients by helping them regain independence and improve their quality of life-all in the comfort of their own homes.As a Home Health OT, you'll be responsible for evaluating patients, developing individualized treatment plans, and implementing interventions tailored to support each patient's goals, whether it's increasing mobility, mastering daily living skills, or ensuring a safe home environment. You'll work collaboratively with a multidisciplinary team including nurses, physical therapists, and speech-language pathologists to ensure comprehensive care.Key Responsibilities:Conduct initial and ongoing assessments to determine patients' functional status, needs, and goalsDevelop and implement customized therapy plans to support rehabilitation and independenceProvide training and education to patients, families, and caregivers on adaptive techniques and equipmentMonitor progress and adjust care plans as needed to achieve desired outcomesEnsure accurate and timely documentation in compliance with regulatory standardsCommunicate effectively with the care team to coordinate treatment and improve outcomesRecommend and assist with the acquisition of adaptive equipment and home modifications to improve safetyParticipate in discharge planning and provide guidance on continued care or community resourcesQualifications:Licensed Occupational Therapist in the state of Indiana (required)Graduate of an accredited OT programPrior experience in home health preferred but not requiredStrong communication, critical thinking, and time management skillsAbility to work independently in a home-based settingWhat We Offer:Competitive pay and mileage reimbursementFlexible schedulingSupportive team and clinical leadershipOpportunity to build strong, lasting patient relationshipsIf you're passionate about patient-centered care and want to make a difference outside the traditional clinical setting, we'd love to hear from you!
