Our Mission As a faith-based organization, QueensCare provides, directly and in collaboration with others, accessible quality healthcare for low-income individuals and families residing in Los Angeles County. QueensCare is a 501 (c)(3) nonprofit organization originally founded by the Franciscan Sisters of Sacred Heart, whose mission of compassion and accessible healthcare continues today. QueensCare provides screening, treatment, education, and preventive and direct care to Los Angeles area residents in need. Beyond that, we also give charitable grants to other non-profits making a difference in the City of Angels and fund scholarships for those entering medical fields with an interest in working in underserved communities. About the role Under the direct supervision of the Associate Director, Community Partnerships, the Lead Community Health Worker will assist the Associate Director with supervising Community Health Workers, coordinate scheduling, and help deliver comprehensive community health and wellness coordination to patients at various locations, ensuring accessibility to quality care for underserved populations. This role requires a combination of excellent direct patient care skills and team management skills. Responsibilities include assisting with scheduling and supervising Health & Faith Partnership Sites, providing direct patient case management and healthcare access coordination, and leading a collaborative and effective culture for the Health & Faith Partnership team. The Lead Community Health Worker will also ensure that Community Health Workers are up to date on needed trainings to support quality patient support initiatives. What you'll do

ESSENTIAL JOB RESPONSIBILITIES

Oversight of Community Health Workers (CHWs) Supervisory Responsibilities Coordinates the daily work of Community Health Workers to ensure efficient service delivery and high-quality patient support. Prepares daily staff schedules, assigns work, coordinates meal and rest break coverage, and adjusts assignments based on operational needs. Reviews and approves timecards, manages day-to-day performance in accordance with organizational policies, and escalates performance concerns and exceptions to management. Monitors staff performance, provides coaching and guidance, reinforces established procedures, and communicates operational concerns to management. Assists with onboarding, orientation, training, and ongoing competency development of Community Health Workers, including required program and regulatory training. Facilitates communication between staff, management, healthcare providers, and community partners to support daily operations and service delivery. Coordinates site visits, community outreach activities, service site readiness, and operational support for new and existing service locations. Documents operational activities, service site issues, and recommendations, and escalates significant concerns to management. Community Health Worker Practice and Patient Support Provides comprehensive case management and healthcare access for patients, including individuals who are housed or unhoused, sick, or injured Performs basic health screenings, including blood pressure, vision, hearing, and osteoporosis screenings. Assesses social determinants of health and coordinates referrals to the Community Health Nurse (CHN) and other internal and external resources. Tracks patient progress, conducts follow-up, and supports patient self-management and continuity of care. Assists patients with scheduling healthcare appointments, accompanies patients to appointments as appropriate, and conducts follow-up to support care coordination. Delivers health education and outreach presentations at community sites and health fairs using culturally responsive approaches. Conducts outreach activities to increase community awareness and engagement with program services. Assists patients with medical insurance enrollment and access to available healthcare resources. Maintains productivity standards and accurate, timely documentation in the Electronic Health Record (EHR). Provides crisis intervention and follows established reporting protocols. Maintains compliance with CDC infection prevention standards, Los Angeles County requirements, and organizational policies for equipment, vaccine management, and documentation. Collaborates with internal programs and community partners to improve access to services and continuity of care. Travels regularly to partner sites throughout Los Angeles County to provide community-based services. Performs other duties as assigned. Policies and Performance Supports and implements the organization’s vision, mission, and values. Complies with and promotes adherence to organizational policies and procedures. Determines priorities and methods to complete daily tasks effectively, ensuring all responsibilities are met promptly. Provides all job functions professionally and courteously. Fosters and promotes a culture of service excellence and accountability. Attends training and educational sessions as required. Must be willing and able to work all business hours including evenings and weekends to attend health fairs/events as needed. Must be willing and able to work at all locations as needed. Qualifications

EDUCATION:

High School Diploma or GED equivalent. Bachelor’s degree in Public Health, Health Administration, Social Work, and or other related health sciences preferred. CHW certification preferred.

EXPERIENCE:

Experience working with adults, children, and families from diverse social, cultural, educational, and economic backgrounds. A minimum of two years of experience working in a health setting, community setting, supervisory experience preferred.

KNOWLEDGE:

Knowledgeable about community resources appropriate to the needs of community members. Demonstrates proficiency in computer applications such as Microsoft Suite, Outlook, MyTurn, EHR (Penelope preferred), and CAIR.

SKILLS:

Works well with children, adolescents, and adults. Organizational skills and attention to detail to manage administrative tasks efficientlY. Strong interpersonal and communication skills; ability to interact compassionately with patients of diverse backgrounds. Demonstrates a self-directed mature, disciplined, and tactful approach to fulfilling job duties. Demonstrates excellent communication skills, both oral and written, preferably bilingual (Spanish/Korean- oral). Ability to obtain CPR/First Aid certification and provide standard first aid care.

Specific vision abilities required by the job include close vision, color vision, and ability to adjust focus. Ability to provide standard first aid care.

The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. However, there are many times when there is a high ambient background noise of phones and multiple conversations. Must be able to screen out the background noise to concentrate on the work at hand.
