Pharmacist, SMG Primary Care Clinical Pharmacy Specialist

Sentara is hiring a Pharmacist (Full-time) Clinical Specialist position for the Primary Care Medical Groups. Location: Hybrid. Local travel and on-site work at primary care clinical locations will be required. Remote telehealth work will be offered at the manager's discretion. Status: FT (40 hours per week) Mon-Fri Shift: Days, 8 hr Shifts Hours: 8:00am-4:30pm or 8:30am-5:00pm *This Hybrid position requires 2-3 days per week onsite each week at one or more primary care practices. As a Pharmacist on the Ambulatory Clinical Support Team, the Clinical Pharmacy Specialist will be responsible for chronic disease management under collaborative practice agreements with 5-10 primary care providers (depends on panel size), including disease states like hypertension, diabetes, hypercholesterolemia, obesity, and more. They will provide support for our Nurse Clinics as well as direct patient care via face‑to‑face visits with patients in the practice, telehealth visits, and follow‑up phone calls. The Pharmacist is helping to develop and implement a new care model and several new service lines. The Clinical Pharmacy Specialist participates in all aspects of the medication‑use system while delivering comprehensive and personalized pharmaceutical care to patients in their designated areas. This role assumes leadership and accountability for specific clinical pharmacy areas, demonstrates advanced skills and expertise, and obtains advanced certification in their practice area if available.
