This position is classified as Non-Exempt and reports to the Director of Rehabilitation and the Facility Administrator.Core responsibilities include:Performing comprehensive evaluations and reassessments in accordance with physician orders and California standards of practiceDeveloping, implementing, and modifying individualized plans of care with measurable functional goalsDelivering skilled therapeutic interventions, both individual and group, to restore function and reduce impairmentMonitoring patient progress and adjusting treatment interventions based on clinical presentation and objective outcomesSupervising and directing Physical Therapist Assistants in accordance with California state law and professional regulationsProviding appropriate oversight to rehabilitation aides, students, and volunteers as assignedCommunicating evaluation findings, progress updates, and discharge outcomes to physicians, interdisciplinary team members, patients, and familiesDeveloping and instructing patients and caregivers in home exercise programs, safe use of assistive devices, orthotics, and adaptive equipmentCompleting timely, accurate, and compliant documentation consistent with federal, state, payer, and organizational requirementsParticipating in MDS assessments, care conferences, care plan development, and quarterly reviewsSupporting discharge planning, transition-of-care coordination, and community reintegration recommendationsParticipating in quality assurance, performance improvement, utilization review, and risk management initiativesMaintaining compliance with HIPAA, patient rights, OSHA, infection control, and all applicable California state practice standardsTreating all residents with dignity, respect, and compassionate professionalismPhysical requirements include the ability to assist with patient transfers, mobility support, repositioning, prolonged standing and walking, bending, reaching, and lifting with or without assistive devices.
