Home Health Baylor Registered Nurse

Position Overview As a CenterWell Home Health Nurse reporting to the Clinical Manager, you will provide personalized, one‑on‑one care in patients’ homes to help them regain independence and support recovery. Schedule Thursday through Monday – Full time, 40 hours per week. What You Will Do Provide skilled nursing care in patients’ homes, including medication administration, wound care, and patient assessments. Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals. Monitor patients’ conditions and report changes. Educate patients and families on disease management, medication, and treatment options. Maintain accurate records and coordinate with other care team members. Report patient care and progress to the physician and Clinical Manager. Oversee the work of Home Health Aides and monitor documentation completion. Required Qualifications Med‑surg, ICU, ER, or acute care experience. Current and unrestricted Registered Nurse licensure. Current CPR certification. Experience collaborating with a team of healthcare professionals. Valid driver’s license, auto insurance, and reliable transportation. Preferred Qualifications One year of nursing experience. Home Health experience. Compensation Pay per visit/unit: $44.55 – $61.28. Annual range (40 hours per week): $69,500 – $95,600. Additional Requirements TB screening is required. Valid state driver’s license and personal vehicle liability insurance are required.
