Weekend Baylor RN Registered Nurse - Home Health

Empath Home Health , a member of Empath Health is seeking a compassionate Weekend Baylor RN Registered Nurse - Home Health to work with our Home Health team to deliver individualized care to patients in their homes throughout North Hillsborough & East Pasco counties (North Tampa, Dade City, Land O'Lakes, and surrounding areas).

Locations: Home health visits in North Hillsborough & East Pasco counties (including North Tampa, Lutz, New Tampa, Tampa Palms, Wesley Chapel, Land O’ Lakes, Dade City, San Antonio, and surrounding areas).

Schedule: This is a Weekend Baylor role requiring full weekends . You decide which schedule below fits your life!

• 4 days (Friday-Monday, Thursday-Sunday, OR Saturday-Tuesday)
• 3 days (Friday-Sunday OR Saturday-Monday)

What You’ll Do

• Perform comprehensive patient assessments, including OASIS assessments at required intervals
• Develop, initiate, and revise individualized plans of care as patient needs evolve
• Deliver skilled nursing services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless, holistic care

What You’ll Need

• Graduate of an approved nursing program with an active Florida RN license
• Prefers BSN or MSN degree
• Minimum of 1 year of direct patient care experience; prior home health RN experience strongly preferred
• Prefers experience using Homecare Homebase (HCHB) EMR
• Strong assessment, critical thinking, and patient education skills
• Willingness to participate in weekend and holiday on-call rotations
