Title
Physical Therapist - Outpatient

Schedule
- Schedule options: Choose five 8-hour days or four 10-hour days
- Patient care hours: 40 hours/week

Assignment Details
- Contract Length: 13 weeks
- Guaranteed Hours: 40 hours/week

Requirements
- Minimum experience: 1 year
- Required certifications: BLS
- EMR: Epic experience required

Roles & Responsibilities
- Evaluate and treat patients with orthopedic and neurological conditions.
- Develop and implement personalized treatment plans.
- Utilize modalities and treatment techniques for rehabilitation, including manual therapy and gait training.
- Collaborate with a multidisciplinary team to ensure comprehensive patient care.
- Maintain accurate documentation and patient records as per facility standards.

Compliance / Notes
- Flu vaccine required (medical/religious exemptions allowed).
- Background checks and compliance with relevant policies apply.
- Requested Time Off (RTO): Recommended maximum of 7 days per 13-week contract.
