Field-Based RN Case Manager: Wound Care & Infusion
Adaptive Home Health, LLC
Adaptive Home Health, LLC is seeking a Registered Nurse Case Manager to deliver skilled home health care. You will own patient assessments, document visits, manage OASIS, and advocate for patients and families in a field-based setting. You will coordinate care with physicians and the care team, drive timely follow‑ups, and supervise LVNs/HHAs as applicable. Expect 30 productivity points per week with a full‑time schedule Monday–Friday and some weekends as needed. #J-18808-Ljbffr Adaptive Home Health, LLC
