Home Health RN Case Manager

Friendship Village

Friendship Village seeks a compassionate Home Health RN Case Manager to provide coordinated, in-home skilled nursing care for senior residents in our faith-based, not-for-profit communities. In this role, you will complete comprehensive assessments, develop and update individualized care plans, manage medications and treatments, and ensure regulatory-compliant documentation. You will collaborate closely with physicians, therapists, families, and the interdisciplinary team to support wellness, safety, and dignity, while building meaningful, long-term relationships in a mission-driven, resident-centered environment.
