Your Opportunity We are seeking a Physical Therapist [PRN] whose primary responsibility is their direct clinical skills, documentation skills, and the compliance to all professional standards. Key Responsibilities Responsible for the evaluation and treatment of residents who demonstrate disorders of musculoskeletal deficiencies, neurological conditions, balance disorders, and transfer/gait deficits. Evaluation and treatment will only be conducted after a written order is received from the physician. Responsible for the screening of residents of the community as follows: New Community residents: Within 24 hours of admission Long term residents: Upon receipt of a "significant change" form and/or upon referral from nursing. Attend all required community meetings and in-services, including Medicare meetings, family meetings, care plans Qualifications Minimum Education Required: Masters of Science Degree or Bachelor of Science Degree in Physical Therapy.
