Step Up Rehab is the in‑house therapy provider at Northbank Center for Rehabilitation and Healing located at 5125 N Main St, Jacksonville, FL 32208 and Lakeside Center for Rehabilitation and Healing located at 13956 Lakeside Dr, Jacksonville, FL 32210. Work inside the dedicated therapy gym within either of these skilled nursing facilities, helping residents regain function and independence through skilled, compassionate care. Physical Therapist – Skilled Nursing Facility (SNF) PRN POSITION Increased rates when medical benefits are waived. This position is exclusively for professionals licensed in the state of Florida as a PT. All services are provided in a Skilled Nursing Facility (SNF). Why Step Up Rehab? Position Summary Provide skilled PT‑related therapy services to residents in a Skilled Nursing Facility while collaborating with the interdisciplinary team to improve safety, function, and quality of life.
