Physical Therapist PT PRNs Needed Cedar Hill Nursing and Rehab Center, Jacksonville, Florida. We are looking for a committed and patient-focused Physical Therapist to join a progressive therapy team. This role involves developing, recovering, or maintaining daily living and work skills for individuals with physical, mental, or developmental conditions, providing client-centered services to promote confidence and independence. Physical Therapist Responsibilities Evaluate, plan, and administer treatment to residents and patients with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing. Assure all treatment is delivered in accordance with an established plan of care. Provide clinical support and instruct patients, families, and caregivers. Monitor patient response to treatment intervention. Complete required forms and documentation in accordance with company policy. Must be able to pass Level 2 fingerprinting with an Eligible status.
