Description:
We are seeking a compassionate and motivated Physical Therapist Assistant to provide in-home therapy services under the direction of licensed Physical Therapists.
This position offers flexibility, independence, and the opportunity to build meaningful relationships with patients while helping them improve mobility, safety, and quality of life in their own homes.
Whether full-time or part-time hours are desired, you will be supported by a collaborative team that values excellent patient outcomes and work-life balance for clinicians.


Responsibilities:
Provide skilled therapy services in patient homes.
Carry out established treatment plans involving therapeutic exercise, gait training, balance activities, and functional mobility intervention.
Monitor and communicate patient progress and changes to supervising PT.
Encourage patient participation and reinforce goals.
Complete timely and accurate documentation.
Collaborate with therapists, nurses, caregivers, and other care team members.
