Clinical Manager - Home Health

Salary Range: $120,000.00 To $140,000.00 Annually Clinical Manager – Home Health – Chantilly Office 100% office-based position (no exceptions) Duties Accountable for branch clinical outcomes and customer satisfaction Ensures survey readiness and successful VDH, Joint Commission, and payer audits Oversees infection prevention and control compliance Monitors clinician productivity, utilization, and case mix Reviews hospitalization trends and implements corrective action plans Ensures timely completion of OASIS assessments and documentation Maintains appropriate staffing levels and clinician productivity Oversees branch on-call operations and clinical support Drives staff engagement, retention, development, and performance Participates in budget planning and expense management Ensures compliance with company policies, HIPAA, OSHA, and regulatory requirements Requirements Active Virginia RN license in good standing Minimum 2 years of home health leadership experience Minimum 2 years as a field RN in home health (no exceptions) Strong knowledge of OASIS, PDGM, Medicare reimbursement, and quality measures Experience managing interdisciplinary clinical teams Strong leadership, coaching, conflict-resolution, and communication skills Homecare Homebase (HCHB) experience preferred KPIs Medicare Star Ratings: Outcomes and HHCAHPS Potentially Preventable Hospitalization Rate Visits Per Episode Employee Retention 360-Degree Performance Reviews Timely OASIS and Documentation Completion #J-18808-Ljbffr Five Star Home Health Care LLC
