Job Location: Corsicana - Corsicana, TX 75110 Position Type: Full Time Job Title: Resident Care Coordinator – Assisted Living/Memory Care Reports To: Executive Director/Nurse Job Summary: The Resident Care Coordinator for Assisted Living/Memory Care is responsible for the direct supervision of resident care staff and ensuring high-quality, person-centered care is delivered consistently across all shifts. This role serves as the primary point of accountability for staff scheduling, on-call coverage, and filling vacant shifts to maintain safe staffing levels at all times. The Coordinator oversees daily care operations, ensures individualized care plans are followed, and acts as a key liaison between residents, families, nursing, and the Executive Director. Key Responsibilities: Resident Care Supervise caregivers and med aides during assigned shifts, ensuring resident care tasks are completed accurately, safely, and in accordance with individualized care plans. Conduct regular rounds to monitor the quality and delivery of resident care, identifying and addressing concerns with staff in real time. Monitor changes in residents’ physical or cognitive condition and ensure prompt communication with the charge nurse, medical providers, and family members. Review and update individualized care plans in collaboration with nursing staff, ensuring all caregivers are trained on and adhering to each resident’s specific care needs and preferences. Train and coach care staff in person-centered and dementia-specific care techniques, including redirection, validation, behavior management, and documentation best practices. Serve as a key point of contact for families regarding care delivery, resident status, and care plan updates, ensuring concerns are addressed promptly and compassionately. Communicate with nursing any resident changes in condition, or care concerns. Scheduling, On-Call & Staffing Coverage Build, manage, and publish staff schedules for caregivers, med aides, and nursing personnel, ensuring 24-hour coverage that meets resident care needs, required staffing ratios, and evacuation support requirements. Serve as the primary on-call contact for all shift vacancies and call-offs. Respond promptly to coverage needs, make every reasonable effort to fill vacant shifts, and step in personally when necessary to ensure resident care is not compromised. Assist with job postings, applicant screening, interview coordination, and new hire communication. Support onboarding by verifying credentials, TB tests, background checks and HHSC required training. Promote a safe environment by ensuring ongoing training; such as fall-prevention strategies, medication management practices, abuse and neglect and behavioral risk mitigation. Foster a warm, compassionate environment where dignity and quality of life are prioritized. Serve as the main point of contact for scheduling matters, time-off and staffing concerns. Monitor attendance, timecard accuracy and Paycom compliance. Maintain and distribute current on-call schedules, staffing rosters, and emergency contact lists; ensure all team members understand on-call expectations and procedures. Assist with performance reviews and corrective action documentation. Communicate with the nurse changes in staffing. Regulatory Compliance Maintain compliance with all state and federal regulations regarding assisted living and memory care services and documentation standards. Keep staffing reports and documentation for surveys, audits and incident investigations. Qualifications 3+ years of experience working in an assisted living or memory care or dementia care setting. Strong understanding of Alzheimer’s and other dementia-related conditions, including common behaviors and interventions. Dementia-specific certification (e.g., CDP – Certified Dementia Practitioner) preferred or willingness to obtain. Demonstrated ability to lead, train, and support a team of caregivers. Outstanding communication and interpersonal skills, especially with families and residents experiencing cognitive decline. CPR and First Aid certification and Medication delegation required. Work Environment: This position involves regular presence on the floor supervising care delivery and interacting directly with residents who have cognitive impairments. The Resident Care Coordinator must be available on-call as scheduled to respond to staffing emergencies and vacant shifts. The role requires the ability to remain composed under pressure, manage competing priorities, and exercise sound judgment in time-sensitive situations. Physical requirements include standing, walking, and occasional lifting (up to 35 lbs).
