Family Nurse Practitioner - AVALA Care - Full-Time - RO49

Summary The Family Nurse Practitioner is responsible for caring for patients, maintaining accurate and current patient records and scheduling as well as administering follow-up appointments to patients as required. They work as a team with our nurses/medical assistants and physicians in delivering premium care to patients. Essential Duties and Responsibilities Conduct comprehensive patient assessments including medical histories and physical examinations Diagnoses and manage acute and chronic health conditions Develop and implement individualized treatment plans in collaboration with supervising physicians Order, perform, and interpret diagnostic tests and laboratory results Prescribe medications and therapies in accordance with state regulations Educate patients on disease prevention, treatment plans, and medication management Document all patient encounters accurately in the electronic health record (EHR) Coordinate care with physicians, specialists, and other healthcare professionals Monitor patient progress and modify treatment plans as necessary Ensure compliance with HIPAA, infection control, and organizational policies Participate in quality improvement initiatives and continuing education Perform other duties as assigned by leadership The above statements are only meant to be a representative summary of the major duties and responsibilities performed by incumbents of this job. Professional/Regulatory Requirements 3 years of Family Nurse Practitioner experience preferred Master’s or Doctoral degree in Nursing from an accredited program Current certification as a Family Nurse Practitioner (FNP-C or FNP-BC) Active and unrestricted Louisiana APRN license Prescriptive authority and DEA registration Current BLS certification (ACLS preferred) Skills Ability to communicate effectively in English, both verbally and in writing.
