RN Case Manager

A-Line Staffing is hiring an RN Case Manager for an onsite contract-to-hire opportunity in Grand Junction, CO. This role focuses on assessing high-risk members, coordinating care across providers, and ensuring safe, cost-effective care transitions. Schedule / Details Schedule: Monday-Friday, 8:00 AM-5:00 PM Hours: 40 hours/week (40 guaranteed) On-call: Potentially 1 day/week and 1 weekend/month (not guaranteed) Work setting: Onsite Counties covered: Mesa County Mileage reimbursement: $0.43/mile Key Responsibilities Perform comprehensive assessments of high-risk members Partner with primary care providers to build and execute individualized care plans Continuously reassess and direct members to appropriate, cost-efficient levels of care Arrange and coordinate care transitions with physicians, social workers, discharge planners, and other stakeholders Engage specialty resources as needed to resolve complex situations Track and document clinical, functional, and fiscal outcomes Identify health promotion and prevention opportunities Advocate for patients while protecting privacy and confidentiality #J-18808-Ljbffr
