Per-Diem Nurse- Allergy/Immunology Ambulatory Clinic

Utilizes the nursing process for assessment, planning, implementation, and evaluation to provide safe, therapeutic care. Functions as a member of the interdisciplinary and nursing teams; carries out established nursing procedures and performs related duties as required; performs nursing care for patients with more complex needs with assistance and supervision. Assists with the management of patient care administered by other members of the nursing team. 20% Assessment Identifies the needs and responses of patients to medical regimen and treatment through observation of symptoms, emotional state, and cultural and a socioeconomic background. Able to collect, process, and analyze data. Recognizes and reports specific physical and emotional conditions/needs/concerns of the patient to appropriate members of the health care team.Alerts a provider of unusual observations of patients to facilitate immediate attention, e.g. anaphylactic emergency, shortness of breath, chest pain Reports emergency or unsafe patient care conditions on the Incident Reporting System and to the management team. 8% Planning Works cooperatively and participates with the health care team to discuss patient's alternatives to health care services and to develop short- and long-range goals to meet the patient's needs. Plans as appropriate for the needs of patient using knowledge of physical and emotional processes as well as cultural and socioeconomic backgrounds. 40% Implementation/Evaluation Provides individualized nursing care which responds to the growth and developmental needs of patients and demonstrates an understanding of the range of treatment required by the patient. Demonstrates ability to provide appropriate care based on the needs of a specific individual, including that individual's age.Check age-specific group(s): Neonatal (Birth - 1 year) Pediatric (1 - 12 years) Adolescent (13 - 18 years) Adult (19 - 64 years) Geriatric (65+ years) Sets up and maintains cleanliness of exam rooms including periodic exam room disinfection and organization and removal of expired supplies. Performs intake process and engages and provides for patient's physical and emotional privacy. Provides in in reach and outreach to support practice quality initiatives and to support the model of population management. Prepares for and instructs patients about specific diagnostic and therapeutic procedures and treatments. Assists as needed with diagnostic and therapeutic procedures and treatments, such as vaccines and injections, PRP, Allergy Testing. Obtains specimens as directed Use appliances and equipment correctly and maintains safe practice. Evaluates and documents patients' responses to nursing care in the electronic medical record, including pre/post procedures and treatments.Uses knowledge of cognitive, physical, emotional and chronological maturation process in evaluating care for all ages of patients. Practices basic assessment and applies communication skills for the purpose of patient care and education. Administers and documents the administration of medication while maintaining current knowledge of pharmacology and competency in modes of administration. Documents telephone messages and chart notes in a thorough and grammatically correct manner. Responds appropriately in emergencies.Is knowledgeable about the contents of the code blue cart and/or the emergency kits, and can initiate and assist in CPR.Understands the procedures for medical emergencies, code blue and other emergency/disaster situations. Participates in safety drills. Complies with all Ambulatory Services and Medical Center policies and procedures for infection control, safety, administrative and clinical practice.Complies with activities mandated by JCAHO/Title 22/NCQA. Completes annual competency testing as specified by Ambulatory and the individual practice. Maintains annual Allergy Competency Certification 10% Coordination/Continuity/Communication A. Organizes activities based on the needs of patients while maintaining the effective flow of patients through the practice.Adjust clinical support activities to reflect changes in priorities during clinic sessions. 5% Advocacy/Education Participates in identifying and documenting the educational needs of patients. Provides patients with the necessary information to make informed decisions utilizing available resources and making referrals appropriately. Informs patients/families about services available at UCSF Medical Center and in the community.Utilizes and maintains updated educational resources. Assists providers with providing patients with EMMI and other educational materials. Provides and reinforces patient health teaching by utilizing language which patient and family can understand. Utilizes and documents interpreting services as appropriate. Verifies patient/family understanding through return demonstration, either verbal, physical or written. Utilizes available educational resources.Participates in review and/orrevision of educational materials. 5% Performance Improvement Participates in Performance Improvement (PI) activities to include ongoing health care maintenance support Makes recommendations regarding PI topics or problems; and assists in data collection. Reports and follows up on issues that affect environment and safety. Maximizes resources by seeking assistance from appropriate support, effectively using supplies, making suggestions for improved approaches, assists in cost effective practices. 2% Self-Development/Professionalism A. Attends classes, conferences, workshops and staff meetings applicable to practice needs.Shares information from programs attended. B. Attends and participates in staff meetings. Complies with work related policies, including license renewal, annual PPD, CPR certification, use of benefit time, and dress code including identification policy. Completes all required local and Ambulatory competencies as well as online training. Refrains from using the internet or cell phone during normal work hours. Contributes to the orientation and learning experiences of other members of the health care team. Complies with departmental scheduling guidelines, i.e. starts work at times scheduled, completes work in a timely manner. 10% Administrative/Clerical Duties Greets and welcomes patients making eye contact and utilizing AIDET standards. Assists with scheduling nurse visit appointments providing appropriate instructions to patients. Discusses practice policies and procedures with patients and referring physicians. Answers questions about provider schedules and acts as a resource to other medical center practices and ancillary service administrative staff. Triages telephone calls, screening for emergencies and routing all messages, requests for same day appointments, refill requests, etc. to the appropriate box. Covers Apex in-baskets and phone messages when other members of the team are absent. Demonstrates an ability to adjust priorities as required for smooth operation of the practice and notifies the clinical staff. Provides administrative support to providers in coordination of patient care (i.e. sending patient letters, educational materials, results of lab tests, etc.) Assists in maintaining current filing and scanning. Assists in chart prep for upcoming clinics Required Qualifications Minimum two years of recent Registered Nurse clinical experience in an Ambulatory Clinic Setting Minimum one year of recent Registered Nurse clinical experience in an OHNS or Allergy/Immunology Ambulatory Clinic Minimum of one year experience with environmental allergy testing and immunotherapy, including skin testing and immunotherapy preparation and administration This position requires flexibility to orient and work at Redwood Shores Specialty Care Clinic and Berkley Outpatient Center Preferred Qualifications Experience with Quality Improvement projects Experience with Red Cap Experience with PRP Required Licenses/Certifications Current licensure as a Registered Nurse in the State of California.
