Your next nursing opportunity starts here. We are seeking an experienced Registered Nurse (RN) with OASIS proficiency for a 13-week full-time home health contract. This role is designed for a detail-oriented clinician who excels in direct patient care, thorough documentation, and effective time management within a structured schedule. Schedule:

• Full-time, Monday through Friday
• On-call availability may be required

Qualifications:

• Hold an active RN license
• Minimum of 1 year of recent experience in home health nursing
• Demonstrated OASIS documentation proficiency
• Familiarity with HomeCare HomeBase (highly preferred)
• Ability to manage daily patient visits and meet structured expectations
• Possess reliable transportation and a valid drivers license

Primary Responsibilities:

• Deliver skilled in-home nursing care and clinical interventions
• Complete accurate, timely OASIS assessments and documentation
• Work closely with physicians and interdisciplinary teams on customized care plans
• Perform wound care, medication administration, patient education, and symptom monitoring
• Communicate patient condition changes and updates to the clinical team
