Job Responsibilities

• Conduct comprehensive physical therapy assessments and evaluate patients' medical histories to determine individualized care needs
• Develop and implement customized treatment plans focused on rehabilitation goals and functional improvement
• Deliver therapeutic exercises, manual therapy techniques, and training programs to restore mobility and enhance patient functionality
• Provide patient and family education on injury prevention, wellness strategies, home safety, and recovery expectations
• Monitor, document, and track patient progress throughout the rehabilitation process, modifying treatment plans as needed
• Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals to ensure comprehensive, coordinated care
• Maintain accurate, timely patient documentation and medical records in compliance with HIPAA, state, and federal regulatory standards
• Travel to patients' homes throughout the service area to provide in-home physical therapy services

Job Requirements

• Current, unrestricted Physical Therapist license in the state of Kentucky (or eligible for licensure)
• Master's degree or Doctorate in Physical Therapy (DPT) from a CAPTE-accredited program
• Minimum of nine (9) months of clinical experience as a licensed Physical Therapist
• Home health care or community health experience strongly preferred
• Current CPR/BLS certification (must be maintained throughout employment)
• Valid driver's license with reliable transportation and ability to travel to multiple patient locations
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal skills with patients, families, and healthcare team members
• Proficiency with electronic medical records (EMR) and documentation systems
• Ability to work independently while managing a home health caseload
