Physical Therapist (Home Health)
Location: Elizabethtown, PA (Office Location)
Coverage Area: Dauphin County, PA
Position Type: Full-Time
Schedule: Monday - Friday | 8:00 AM - 5:00 PM
Compensation: $108,000 - $132,000 annually (estimated based on full-time status)
Pay Model: Pay Per Point
Sign-On Bonus: Up to $20,000 (paid in installments)
On-Call Requirement: Yes


Position Overview

Wheeler Staffing Partners is seeking a compassionate and skilled Physical Therapist to join a Home Health team in the Elizabethtown, Pennsylvania area. This role is responsible for delivering high-quality, patient-centered care in the home setting, focusing on improving mobility, strength, and overall functional independence.


The ideal candidate is committed to clinical excellence, patient advocacy, and collaborative care, working closely with an interdisciplinary team to drive positive patient outcomes.


Key Responsibilities

• Provide skilled physical therapy services to patients in their homes
• Assess patient needs and develop individualized treatment plans
• Implement therapeutic interventions to improve mobility, strength, and function
• Monitor patient progress and adjust care plans as needed
• Collaborate with physicians and interdisciplinary team members
• Educate patients and caregivers on treatment plans and home exercise programs
• Maintain accurate and timely documentation in compliance with regulatory standards
• Participate in on-call rotation as required

Qualifications

• Minimum of 9 months of experience as a Physical Therapist (home health or community-based experience preferred)
• Strong clinical assessment and patient care skills
• Ability to work independently in a home health environment
• Excellent communication and interpersonal skills

Education Requirements

• Master's degree in Physical Therapy from a CAPTE-accredited program

Licenses and Certifications

• Active Physical Therapist license in the state of Pennsylvania
• Current CPR certification (must be maintained throughout employment)
• Valid driver's license with reliable transportation
