SUMMARY
The Medical Records Clerk provides the necessary administrative support to ensure Residential medical records are maintained and services are documented in accordance with applicable regulations and accreditation standards.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Maintain organization, filing, and storage of all Residential treatment medical records.
- Assist in medical records audit/review preparation as needed.
- Assist with the creation, configuration, and maintenance of user accounts within the electronic health record (EHR).
- Update youth records as needed within the EHR, including psychiatrist, clinician, unit changes, and uploading new/updated youth photos.
- Prepare medical records/documents as youth are admitted and ensure all intake documentation is received.
- Prepare discharge medical records and other documents as needed for transfer to offsite storage location.
- Process and fulfill applicable medical records requests and respond to subpoenas for medical records in a timely manner and per applicable regulations.
- Complete various tracking spreadsheets (client utilization, incident reports, documentation, etc.) to provide Residential and interagency departments with required data.
- Enter serious incident data for Residential youth in SDCC's internal electronic health record per required timeframes.
- Submit required documentation (serious incident reports, quarterly reports, treatment updates, etc.) to external entities as indicated for all Residential youth per specified and required timeframes.
- Take appropriate measures to ensure confidentiality of protected health information and records.
- Immediately report any unsafe acts, conditions, areas of risk, or accidents in their department and or organization to immediate supervisor.

- Fulfill all contractual and organizational training requirements.
- Perform special projects, assignments and other related duties as required or assigned.
On The Job Mandatory Ongoing Certification Training:
- Maintain all certifications and licenses required for the role, ensuring they remain current
- Complete and stay up-to-date with all mandatory trainings as specified by the San Diego Center for Children, including but not limited to: Residential Required Trainings, False Claims, HIPAA, Safety (Emergency Management, Infection Prevention and Control, Injury Prevention, and Active Shooter Preparation), Privacy and Security, Mandated Reporting, Code of Conduct, Rights of Individuals Served, Professional Boundaries, Cultural Competency, including designated IDEA Trainings
LANGUAGE SKILLS
Ability to read, analyze, and interpret medical records, journals, reports, and legal documents. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community. Ability to effectively present information to management and public groups.
REASONING ABILITY
Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of instructions and deal with several abstracts and concrete variables.

Qualifications
MINIMUM REQUIREMENTS
- High School Diploma or GED Equivalent
- Valid California Driver's License
- Must be 21 Years of Age or Older
- At least 1 Year of experience in Administrative Work, Medical Records, and/or Quality Assurance/Improvement
PREFERRED REQUIREMENTS
- Associate or Bachelor's Degree in a related field
- Experience working with an electronic health record system
