Physical Therapist (PT) - Home Health

Schedule / Job Type

Contract (13 Weeks) | Full-Time | Onsite | Monday through Friday, 8:00 AM - 5:00 PM

Key Responsibilities

• Provide skilled physical therapy services to patients in a home health setting
• Evaluate and treat patients within an assigned territory covering Spokane County and potentially lower Stevens County
• Develop and implement individualized treatment plans based on patient needs and goals
• Document patient progress and clinical notes using HCHB electronic medical records
• Collaborate with interdisciplinary care teams to coordinate patient care

Required Qualifications

• Active Physical Therapist (PT) license in the state of Washington
• Minimum of 12 months of Home Health experience within the past 3 years
• Proficiency with HCHB electronic medical records system
• Valid driver's license and reliable transportation for patient home visits

Preferred Experience / Skills

• Familiarity with Spokane County and surrounding areas
• Strong organizational and time management skills for managing a home health caseload
