The Geriatric Care Manager provides assessments and continuous reassessment and manages comprehensive aspects of care including medical and cognitive challenges of aging in a manner that ensures their dignity, independence and wishes are respected.Essential Duties and Responsibilities:Development of Comprehensive Care Plans and implementation of Care PlansTrack, assess and document observations regarding client condition, changes in preferences and problems/opportunities, medical orders, family requests and other information pertinent to Care Plan.Monitor, Evaluate and report on Client Status, Actions/Interventions, and OutcomesWorks with formal and informal social support systems and families to ensure client's needs are met.Provide guidance, resources, and information to enable families and clients to make informed medical decisions.Manage Electronic Health records for clients including billing.Coordinate resources for client care.Interact with other professionals on the client's behalf this may include, physicians, attorneys, fiduciaries, hospice personnel, and community personnel.Create and maintain referral resources, identification, coordination, management and advocacyAdditional duties and responsibilities as may be assigned by your supervisorRequired Skills, Education and Certifications:(5) Years' experience in community, hospital, hospice or home health case managementBachelor's or master's degree in Healthcare Related Field, preferredCertified Care Manager (CMC) or Certified Case Manager (CCM)Demonstrated knowledge of aging, dementia, and/or disabilities.Experience working with complicated family systems and dynamics.Computer Skills: MS Office, Electronic Health Records and Internet SkillsExcellent customer service and problem-solving skillsExcellent verbal and writing skillsDetail-oriented with the ability to keep up with fast paced processesPhysical Requirements:Ability to lift and carry up to 10-15 poundsAbility to sit, stand and walk for prolonged periods of time throughout the work dayFrequent local travel by personal automobile to client homes requiredAbility to use standard office equipment such as personal computer, laptop, telephone, photocopy machine, etc.Benefits for full time employees:Medical/Dental/Vision InsuranceTouchCare VirtualCareLife InsuranceHealth Savings AccountFlexible Spending Account401(k) MatchingEmployee Assistance ProgramPTO Plan for Non-Exempt EmployeesFlexible PTO Plan for Exempt EmployeesHolidays and Floating HolidaysPet InsuranceTheKey is an equal opportunity employer.
