HPH Float Social Worker

Social Worker – Provides and coordinates psychosocial support to patients and their families across various clinical settings. Qualifications Master’s degree or above in Social Work (CSWE accredited). Grandfathered qualifications: SWs hired before 2009 with degrees in other fields; SWs hired before 4/21/2019 with BSWs. Bachelor’s degree may be considered depending on geographic area (Note: Home Health requires MSW). Current Georgia LCSW license required for Georgia locations. Minimum of 1 year of experience, including social work/counseling internship in health care or hospital setting. Experience with adult and pediatric patient populations; hospice and palliative care experience preferred. Previous experience working with an EMR/EHR system. Experience in children, family, and group modalities. Valid driver’s license and automobile insurance (mobile driver requirement). Responsibilities Perform and document psychosocial assessment. Empower patients/families to negotiate with resource providers. Assist other clinical staff in understanding family dynamics and social/emotional factors. Maintain accurate, timely, and legible documentation. Consult and collaborate with other professionals to promote quality care and continuity. Perform additional duties as assigned. Home Health Specific Participate in development of individualized Plan of Care and case conferences. Observe clinical symptoms, reaction to treatment, and emotional status; report changes. Anticipate barriers to pain and symptom management; provide education. Respect patient’s wishes and stated goals when planning and providing care. Hospice Specific Collaborate with clinicians and volunteers to provide psychosocial support, advanced directive information, and Plan of Care. Attend and participate in interdisciplinary group meetings; facilitate patient‑family meetings. Evaluate bereavement needs; coordinate bereavement staff as needed. Palliative Care Specific Assist the palliative care team with goal setting and advance care planning. Provide emotional support to patients and families. Coordinate educational and outreach services relevant to the social services–palliative care interface. Serve as liaison between inpatient, outpatient, discharge planning, and community services. Florida Keys & North Florida (3A) Addendum May include bereavement counseling, volunteer coordination, and related duties. Recruit, screen, train, supervise volunteers for social services and bereavement. Maintain volunteer records and ensure compliance with documentation standards. Compensation Pay Range: $55,232.15 - $86,300.24 Benefits Sign‑on bonus and benefits effective day 1. Position requires every‑other weekend, one summer and one winter holiday.
