Key Responsibilities\n

• Conduct in-depth assessments to identify and analyze clients’ social, emotional, and environmental challenges.

• Develop and implement personalized service plans that detail specific goals, interventions, and methods for tracking progress.

• Provide individualized counseling sessions to assist clients and their families in navigating various life challenges, including mental health issues, crises, and interpersonal conflicts.

• Facilitate crisis intervention efforts, conducting risk assessments and creating safety plans as needed to ensure client well-being.

• Collaborate with multiple community agencies and organizations to coordinate services and effectively advocate for client needs.

• Engage in community outreach initiatives designed to raise awareness about available agency services and promote engagement.

• Participate actively in weekly case conferences, team meetings, and professional training sessions to enhance team collaboration and service delivery.

Patient Population / Client Focus\n

Our clinical social workers work primarily with a diverse clientele, including individuals and families from various socioeconomic backgrounds facing complex social challenges. Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• Current, active licensure as a Licensed Clinical Social Worker (LCSW) in New York State.

Compensation and Benefits\n
