Heart of Ohio Family Health Centers is seeking a Patient Navigator to engage Medicaid patients and those missing visits. You will schedule care, assist with medications and referrals, and use UnityPHM to text patients. The role requires strong customer service and teamwork within a diverse clinic environment.
