The FNP/PMHNP will evaluate individuals experiencing acute psychiatric symptoms, behavioral health crises, substance-use-related concerns, and co-occurring medical conditions. This provider will perform psychiatric and medical evaluations, establish diagnoses within applicable scope, prescribe and manage medications, order and review appropriate diagnostic testing, develop stabilization plans, and participate in decisions regarding continued observation, discharge, referral, or transfer to a higher level of care. The provider will work collaboratively with Crisis RNs, licensed behavioral health clinicians, BHTs, case managers/care coordinators, peer support specialists, and community healthcare partners to provide safe, timely, trauma-informed, and coordinated care. Psychiatric and Crisis Services Conduct comprehensive and focused psychiatric evaluations. Evaluate individuals experiencing suicidal ideation, self-harm risk, homicidal ideation, psychosis, mania, severe depression or anxiety, agitation, behavioral dysregulation, and other acute psychiatric presentations. Establish and document psychiatric diagnoses within PMHNP scope. Assess psychiatric stability and immediate treatment needs. Develop individualized psychiatric stabilization and treatment recommendations. Initiate, continue, adjust, or discontinue psychotropic medications when clinically appropriate. Monitor therapeutic effectiveness, side effects, adverse reactions, contraindications, and medication interactions. Participate in suicide-risk, violence-risk, safety, and crisis-management planning. Determine when psychiatric presentation warrants continued crisis observation, inpatient psychiatric treatment, emergency intervention, or outpatient follow-up. Provide psychiatric consultation to the multidisciplinary crisis team. Educate patients and authorized family/support persons regarding diagnoses, medications, treatment options, safety plans, and follow-up needs. Medical and Physical Health Services Conduct focused and comprehensive medical histories and physical examinations. Evaluate acute physical-health concerns occurring during behavioral health crisis episodes. Assess chronic medical conditions that may impact stabilization or psychiatric presentation. Diagnose and treat appropriate general medical conditions within FNP scope and competency. Identify medical conditions that may mimic, worsen, or contribute to psychiatric symptoms. Evaluate concerns including hypertension, diabetes, infections, dehydration, respiratory complaints, medication reactions, pain, and other medical conditions. Order, review, and interpret appropriate laboratory and diagnostic testing. Develop medical treatment recommendations and follow-up plans. Determine when a patient's medical needs exceed the capabilities of the crisis stabilization setting. Coordinate transfer to an emergency department, hospital, urgent care, specialty provider, or other medical setting when indicated. Substance Use & Co-Occurring Disorders Assess individuals presenting with substance-use-related behavioral or medical concerns. Evaluate signs and symptoms of intoxication, withdrawal, overdose, medication interactions, and co-occurring psychiatric conditions. Collaborate with Crisis RNs regarding withdrawal monitoring and changes in patient condition. Evaluate whether patients require medical detoxification, inpatient care, residential treatment, crisis stabilization, or outpatient SUD services. Provide medication treatment for substance-use disorders when within professional scope, competency, prescribing authority, credentialing, and organizational privileges. Coordinate with SUD providers and community recovery resources. Incorporate relapse prevention, harm-reduction, recovery-support, and follow-up recommendations as clinically appropriate. Medication Management Complete medication reconciliation upon admission and throughout the crisis episode as appropriate. Review psychiatric and general medical medications. Prescribe medications within applicable FNP and PMHNP scopes. Identify medication duplication, interactions, contraindications, allergies, adherence concerns, and safety risks. Monitor response to new or adjusted medications. Order clinically appropriate laboratory monitoring. Provide medication education and counseling. Coordinate prescriptions with pharmacies and receiving providers. Maintain required controlled-substance and prescribing documentation. Crisis Stabilization & 23-Hour Observation Complete timely provider evaluations after admission. Establish initial psychiatric and medical treatment recommendations. Develop provider orders appropriate to the patient's presenting needs. Reassess patients throughout the observation/stabilization period based on clinical condition. Respond promptly to changes in mental or physical status. Collaborate with nursing staff regarding vital signs, medication administration, withdrawal symptoms, medical concerns, adverse reactions, and behavioral changes. Participate in multidisciplinary case reviews and shift handoffs. Determine when continued crisis stabilization is appropriate versus transfer to another level of care. Participate in discharge-readiness determination and transition planning. Ensure appropriate follow-up recommendations and medication plans are documented before discharge. Emergency Response Identify psychiatric and medical emergencies requiring immediate intervention. Initiate emergency protocols within provider scope. Coordinate with EMS, emergency departments, hospitals, poison control, law enforcement, or other emergency resources when clinically necessary. Provide clinical consultation during behavioral or medical emergencies. Determine when patients cannot safely remain in the crisis setting. Ensure appropriate transfer documentation and provider-to-provider communication when patients require higher levels of care. Integrated Care & Collaboration The FNP/PMHNP will collaborate closely with: Crisis Registered Nurses Crisis Case Managers/Care Coordinators Hospitals and emergency departments Pharmacies and laboratories SUD treatment programs Residential and community behavioral health providers The provider is expected to contribute actively to interdisciplinary treatment planning, clinical consultation, discharge planning, and continuity-of-care efforts. Documentation Responsibilities Complete psychiatric evaluations and medical assessments accurately and timely. Document diagnoses, clinical findings, medications, orders, treatment recommendations, and patient response. Complete progress and reassessment notes. Document clinical rationale for continued observation, discharge, transfer, hospitalization, or other disposition recommendations. Maintain complete and accurate electronic health records. Complete applicable payer authorization and medical-necessity documentation when assigned. Document communication with external providers and receiving facilities. Maintain confidentiality and comply with HIPAA and applicable behavioral health privacy requirements. This position serves individuals who may be experiencing severe psychiatric symptoms, suicidal thoughts, psychosis, agitation, intoxication/withdrawal concerns, medical complications, or other acute behavioral health conditions. The provider must be able to remain calm, clinically focused, and professional in a high-acuity crisis environment. Scheduling may include day, evening, weekend, holiday, and on-call coverage depending on program needs. Requirements Minimum Qualifications Current and unrestricted Arizona RN license or applicable authorization to practice in Arizona. Current national certification as a Family Nurse Practitioner (FNP) . Current national certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP) . Current prescribing authority as required. DEA registration when prescribing controlled substances is part of the position. Current CPR/BLS certification. Ability to meet organizational privileging and payer credentialing requirements. Strong psychiatric assessment and psychopharmacology skills. Strong medical assessment and primary-care clinical skills. Ability to manage complex patients with co-occurring psychiatric, substance-use, and medical conditions. Strong clinical judgment in high-acuity and rapidly changing situations. Excellent documentation and interdisciplinary communication skills. Preferred Qualifications Minimum 2 years of advanced-practice clinical experience preferred. Experience in psychiatric emergency services, behavioral health crisis stabilization, inpatient psychiatry, 23-hour observation, emergency medicine, integrated behavioral health, or community mental health. Experience treating co-occurring mental health and substance-use disorders. Experience with suicide-risk and psychiatric safety assessment. Experience with intoxication and withdrawal presentations. Experience with AHCCCS/Medicaid populations preferred. Familiarity with SMI, SUD, homelessness, social determinants of health, and community behavioral health resources. Experience using electronic health records. Strong experience coordinating with hospitals, crisis teams, pharmacies, and community providers. The successful candidate should demonstrate competency in psychiatric evaluation, medical assessment, psychopharmacology, general medical pharmacology, crisis intervention, suicide and safety assessment, medication management, SUD/co-occurring disorders, emergency recognition, trauma-informed care, interdisciplinary collaboration, diagnostic reasoning, and timely clinical documentation. Scope of Practice The FNP/PMHNP must provide services only within the practitioner's education, certification, demonstrated competency, prescribing authority, organizational privileges, and payer credentialing . Dual certification permits practice within both recognized population focuses but does not eliminate the responsibility to consult, refer, or transfer patients whose medical or psychiatric needs exceed the provider's competency or the capabilities of iCare's program. Candidates must hold active certification and authorization to practice as both an FNP and PMHNP , meet prescribing and credentialing requirements, and demonstrate strong clinical judgment in psychiatric, medical, SUD and crisis presentations.
