Field Care Manager Nurse

Humana

Humana is seeking a Field Care Manager Nurse to coordinate and deliver high-quality care to members in their homes and community settings. In this role, you’ll conduct comprehensive assessments, develop person-centered care plans, and collaborate with physicians, social workers, and caregivers to improve health outcomes. You will educate members on conditions, medications, and preventive care while monitoring progress and adjusting plans as needed. This role blends autonomous field work with virtual collaboration, supporting Humana’s mission to provide accessible, personalized, and value-based healthcare services.
