Case Manager, RN

Summary The purpose of this position is to provide ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual patient needs. The overall goal is to enhance the quality of patient management and satisfaction, promote continuity of care and cost effectiveness through the integration of case management, utilization review, and discharge planning. The Case Manager must be highly organized, detail‑oriented, adaptable to frequent change, and compliant with regulatory and departmental guidelines and policies. Responsibilities Serve patients/families, hospital staff, medical staff, community agencies, and insurance companies by conducting interpersonal relationships that project a positive image of the department and hospital. All contacts with customers are friendly, helpful, and courteous. Qualifications Education Graduate from an accredited school of nursing, BSN preferred. Experience Two (2) years of health care experience as a Registered Nurse. Licensure/Registration/Certification Licensed to practice as a Registered Nurse in the state of North Carolina. Other skills and qualifications Strong assessment and critical thinking skills. Work Environment Office setting in a hospital environment. Physical Requirements Standing, walking, sitting, reaching, talking, seeing, hearing, tasting, smelling, carrying, pushing, pulling, lifting, and moving.
