Minimum Qualifications

• Associate degree in Physical Therapy from an accredited educational program.
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• Arizona PTA license required to start.
• Physical Therapy Assistant checklist required.
• Physical Therapy Assistant competency exam required.
• CPR (Basic) required to start.
• Completed Orientation Documents required.

Assignment Details

• Shift hours: 8 hours.
