Description

• Conduct evaluations for patients in inpatient or outpatient settings and craft personalized treatment plans
• Deliver physical therapy interventions with a focus on mobility, strength, balance, and overall function
• Engage collaboratively with nurses, physicians, case managers, and other rehabilitation specialists to elevate patient outcomes
• Inform patients and their families about therapeutic objectives, discharge strategies, and at-home exercises
• Keep meticulous documentation of evaluations, interventions, and patient progress, adhering to regulatory standards
• Participate in multidisciplinary care rounds and coordinate efforts during interdisciplinary conferences

Education

• Doctorate in Physical Therapy (DPT) from an accredited program or equivalent degree

Certifications

• State licensure as a Physical Therapist
• CPR certification is a plus
• Additional specialized certifications (e.g., geriatric or neurological) are welcomed but not mandatory

Skills

• Prior experience in the field is strongly preferred
