Must-Have: Knowledge of discharge planning and care coordination. Experience in inpatient settings, with ER experience preferred. Proficiency in EMR systems, preferably Epic.

Description: 8-hour shifts, Monday to Friday, with rotating weekends. Candidates must not hold full-time, part-time, or PRN positions with associated facilities. A driver's license is required and will be used to determine eligibility based on proximity requirements. Artificial nails are prohibited in all departments providing patient care. Refusal to comply will lead to contract termination. Responsibilities include discharge planning and care management. The workload involves caseload management of fewer than 40 patients or consults per day. Department-specific orientation will be provided both onsite and online. Cancellation policy allows for shifts to be canceled as specified in the guidelines. BLS certification is a requirement for this position.
