RN - Case Manager Profession: Registered Nurse Specialty: Case Manager Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 16:30) Experience: Minimum 3 years in inpatient hospital/acute care, first-time travelers accepted License: Active RN state license required Certifications: BLS preferred, CPI certificate required Must-Have: Epic charting experienceExperience in inpatient hospital/acute careLCSW 3-year experience accepted Description: The Case Manager RN oversees guardianships, complex medical situations, and placements.This role is responsible for managing social situations that affect discharge planning.The position requires providing and coordinating medical home care, placement, and social issues for safe discharges.Candidates will work in a collaborative environment supported by team leads, managers, and administrative staff.Responsibilities include comprehensive case management and discharge planning.The Case Manager will conduct SDOH screenings, case management assessments, and care coordination.Care coordination referrals to outpatient and post-acute resources are expected.Flexibility to float between different facilities within a reasonable distance is required.Weekends are not mandatory, but additional shifts may be available for those interested.Candidates must be comfortable with required floating and adapt to varying needs of the unit.Compliance with all relevant regulations and standards is crucial. #J-18808-Ljbffr
