Responsibilities:

• Provide direct patient care, including but not limited to, bathing, dressing, grooming, and toileting

• Assist with medication administration, as certified and trained

• Assess and report changes in patient condition to the nurse or physician

• Maintain accurate and detailed records of patient care and services provided

• Work closely with the interdisciplinary care team to develop and implement personalized care plans

• Communicate effectively with patients, families, and healthcare professionals to ensure seamless coordination of care

• Participate in ongoing training and professional development to stay current with industry best practices and standards
