Responsibilities Collaborate closely with psychiatric medical providers and therapists on the patient’s primary treatment team to ensure a cohesive and effective plan of care. Conduct effective assessments and suicide risk evaluations, develop treatment and discharge plans, and lead educational or therapeutic groups. Maintain open lines of communication with patients and their families to involve them in treatment decisions. Coordinate care with outpatient providers during patient admissions and discharges to ensure continuity of care. Facilitate weekly treatment team meetings, creating and updating master treatment plans based on team consensus. Bring a unique clinical perspective to the interdisciplinary team, drawing on training in family systems and social work theory. Qualifications Bachelor’s Degree in Social Work required; Master’s Degree in Social Work highly preferred. State of Minnesota licensure required. Experience in mental health highly preferred. Experience using electronic health records preferred. Proficient in Microsoft Office Products including Word, Outlook, and Teams. Strong written and verbal communication skills. Ability to smoothly and rapidly shift between dissimilar tasks and respond appropriately in times of mental health crisis. Compensation Hourly rate: $27–$35 per hour depending on licensure and experience. Pay rate is determined by skills, experience, education, market conditions, and internal equity.
