We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our evening shift team at Lock Haven Rehabilitation & Senior Living. About the Role:

As an Evening Shift LPN, you will be responsible for providing direct patient care to a diverse population of residents, including those with complex medical and psychosocial needs. You will work closely with our experienced nursing staff to develop and implement individualized care plans, administer medications, and perform various medical procedures as needed.

Responsibilities:

• Provide direct patient care to residents, including taking vital signs, administering medications, and performing wound care

• Develop and implement individualized care plans in collaboration with the interdisciplinary team

• Monitor and report resident progress to the nursing staff and medical director

• Participate in regular care conferences and meetings to ensure continuity of care

• Demonstrate knowledge of infection control principles and follow established protocols

• Maintain accurate and up-to-date resident records and reports
