Description

  • Conduct evaluations and devise individualized treatment plans tailored to patients in both inpatient and outpatient settings
  • Deliver physical therapy techniques aimed at enhancing mobility, strength, balance, and functional ability
  • Work in collaboration with nurses, physicians, case managers, and multidisciplinary professionals for optimal patient outcomes
  • Guide patients and their families through therapeutic goals, discharge strategies, and follow-up exercise regimens
  • Maintain accurate and thorough documentation of evaluations, treatments, and patient progress as per facility regulations
  • Engage in care team discussions and interdisciplinary meetings to facilitate comprehensive care

Education

  • Doctorate in Physical Therapy (DPT) or a related degree from an accredited institution

Certifications

  • Active state license as a Physical Therapist
  • Current CPR certification is preferred
  • Specialty certifications (e.g., in geriatrics or neurology) are a bonus but not essential

Skills

  • Strongly preferred prior experience in a clinical setting
  • Proficiency in treating patients facing complex medical challenges
  • Exceptional communication and collaborative skills
  • Comfortable with digital charting and clinical documentation tools
