Job Summary:

The Palms Care Center, a renowned leader in home health care, is seeking a highly skilled and dedicated Physical Therapist (PT) to join our per diem team in Lauderdale Lakes, Florida. Responsibilities:

• Evaluate patients with a wide range of medical conditions, conducting thorough assessments to develop individualized treatment plans

• Design and implement effective physical therapy interventions, including exercises, stretches, and modalities, to promote patient recovery and independence

• Collaborate with a multidisciplinary team of healthcare professionals to ensure seamless patient care

• Maintain accurate and timely documentation of patient progress and treatment plans

• Communicate effectively with patients, families, and healthcare team members to address concerns and provide education

• Stay current with industry developments and best practices in physical therapy, participating in continuing education and professional development opportunities

Requirements:

• A Doctor of Physical Therapy (DPT) degree or equivalent education and experience

• Current licensure to practice physical therapy in the state of Florida

• Proficiency in electronic medical records (EMRs) and ability to learn our organization's specific systems

• Strong communication and interpersonal skills

• Ability to work effectively in a fast-paced environment and prioritize tasks to meet deadlines
