What You'll Do:
- Coordinate care from admission through discharge
- Advocate for patients and families
- Collaborate with treatment teams and community resources
- Keep plans organized, updated, and compliant
- Bring empathy, organization, and problem-solving to every shift

Requirements
• Required education and experience - Combination of Bachelor's degree in a human services field with at least 1 year of full-time experience serving people with mental illness.
• Preferred education and experience - 2 years of experience in case management or patient care in a healthcare or social services delivery system preferred. Experience in an inpatient care setting is also preferred.
