Ambulatory Nurse - Pediatric Cardiology (BCH-Mission Bay)

Job Description Under general supervision, guided by the policies and procedures of the Medical Center and Ambulatory Services, and in collaboration with providers and administrative management, the Ambulatory Pediatric Heart Center Nurse works autonomously, utilizing the nursing process, to assess, plan, coordinate, implement and evaluate patient care within the ambulatory setting. The nurse will utilize a high level of clinical knowledge to fulfill patient needs, ensure efficient patient care, and provide patient education when necessary. The Nurse Care Coordinator will act as a patient advocate and as a liaison between insurance companies, inpatient/outpatient services, patients/families and all other necessary healthcare services to manage and facilitate efficient and timely coordination of care. In addition, the Clinical Nurse Care Coordinator will assist in the development of patient care standards and practices. The Pediatric Heart Center Nurse Care Coordinator is responsible for coordinating outpatient services for a panel of pediatric cardiology patients and providers in the ambulatory setting. Responsibilities include clinic procedures, outpatient flow management, insurance authorization, home health, DME, medication refills, patient teaching, and overall coordination of timely patient care. The position is 100% onsite, primarily in San Francisco with additional support in Oakland and satellite clinics. Responsibilities Case Management : Contact insurance companies, home health agencies, DME companies, parents, pharmacies, CCS, etc. to coordinate timely care. Return calls to pharmacies and parents to facilitate prescription renewals. Facilitate communication among parents, providers, and support services. Fax and submit paperwork promptly. Assist in creating/reorganizing teaching materials and provide patient education. Clinic Coordination and Support : Use clinical judgment to determine appropriateness of testing, perform regulatory chart audits per Ambulatory Nursing Administration guidelines, perform clinical activities promptly and accurately, and comply with policies for infection control, safety, and administration. Meet TJC/Title 22 requirements, complete annual review and trainings, and document patient assessment and needs in APEX. Patient Medication Management : Coordinate medication authorizations, ordering, and safe administration. Assist with prescription refills and document all interactions in APEX. Coordination of Lab Requisitions and Reports : Assist providers with ordering labs, coordinate requisition slips, send to labs or patients, and follow up. Maintain records, ensure completion of studies, review and distribute results, flag abnormal findings, and discuss with providers as directed. Patient Support : Answer phone calls, emails, CRMS, and in-basket messages. Address patient care needs within scope, escalating to provider when appropriate, record conversations in APEX, facilitate letters for school/employment, contact families after medication changes, coordinate outside resources, and follow up with families. Assessment : Identify patient needs through observation of symptoms, emotional state, and socio-cultural background. Recognize and report physical and emotional concerns, and identify patients suitable for transition education and planning. Planning : Cooperate with the health care team to meet patient needs, review and triage new patient referrals, troubleshoot provider schedules, and use resources such as social work or child life. Coordinate multidisciplinary care for California Children’s Services patients and report to CCS. Implementation : Provide individualized nursing care tailored to growth and developmental needs. Administer medications, maintain competency, comply with policies, and respond to emergencies. Use appropriate equipment for diagnostics and therapeutic procedures, coordinate transition services, and organize transfer to adult care. Coordination, Continuity, and Communication : Organize activities, prioritize patient needs, maintain effective patient flow, communicate via phone, verbal interaction, or written reports, use APEX for labs and appointments, and maintain accurate records. Follow telephone standards, promote positive public relations, and demonstrate cultural sensitivity. Performance Improvement : Participate in PI activities, provide recommendations, assist in data collection, and report issues affecting environment and safety. Self Development / Professionalism : Attend classes, conferences, workshops, and staff meetings; comply with licensing, CPR, PPD, and dress code; volunteer in orienting and training new team members; maintain work time accountability and follow medical center policies. Other Functions & Responsibilities Perform other duties related to patient care activities as required. Qualifications Bachelor’s Degree in Nursing (BSN) Five or more years of pediatric cardiac inpatient nursing experience Experience working in an outpatient/ambulatory setting Knowledge of Pediatric Congenital Heart Disease Strong heart rhythm interpretation experience Strong PIV insertion skills Experience performing Bubble studies Flexibility to orient and work at all UCSF Medical Center locations. Preferred Qualifications Master’s Degree in Nursing (MSN) CCS paneled Experience with an Electronic Medical Record system, EPIC/APEX strongly preferred Microsoft Outlook Experience Licensure / Certifications Current licensure as a Registered Nurse in the State of California.
