Occupational Therapist (OT) – PRN Ascend Home Health | Southern Idaho Ascend Home Health is seeking a compassionate and motivated Occupational Therapist (OT) to join our team on a PRN basis serving patients throughout Southern Idaho. This role offers flexible scheduling and the opportunity to make a meaningful impact by helping patients regain independence, improve daily functioning, and safely remain in the comfort of their homes. Responsibilities: Evaluate and treat patients in their homes according to physician orders and plan of care Develop individualized therapy treatment plans focused on improving daily living skills and independence Educate patients, families, and caregivers on therapy techniques, safety, and adaptive equipment Collaborate with nurses, therapists, physicians, and the interdisciplinary care team Complete timely and accurate documentation Qualifications: Current Occupational Therapist license required Home health experience preferred, but not required Strong communication and organizational skills Compassionate, dependable, and patient-focused Valid driver's license, reliable transportation, and current auto insurance required Position Details: PRN / Flexible scheduling Coverage area throughout Southern Idaho Competitive pay per visit DOE (depending on experience) At Ascend, we are committed to providing compassionate, personalized care while supporting our employees with a positive and collaborative work environment.
