Position Overview

We are seeking dedicated HHAs and CNAs to provide 1-on-1 care in home settings, including clients served through the APD program.

In these roles, care goes beyond basic assistanceyou'll play a key part in supporting independence, maintaining routines, and building meaningful relationships with clients.

Key Responsibilities

Provide personal care and assistance with daily living activities
Support client safety and follow established care plans
Encourage independence and participation in daily routines
Assist with mobility, hygiene, and companionship
Observe and report any changes in client condition
Communicate effectively with the care team and family
Maintain accurate and timely documentation
