Role Overview: This role involves medication management and clinical support for inpatient patients by verifying orders, monitoring therapy outcomes, and ensuring safe, evidence-based medication use. They collaborate closely with physicians, nurses, and other healthcare professionals to optimize treatment plans and prevent medication errors throughout the patient's facility stay.

Requirements:

• Flu Vaccine Required - Medical/Religious Exemptions and Declinations Allowed
• MUST HAVE VA LICENSE IN HAND AT THE TIME OF SUBMITTAL - FIRM
• Bachelor of Science in Pharmacy Required.
• PharmD, RPh required
• EPIC Charting Experience Required
• Skills/Experience Required: IV experience, pain controlled analgesia, antibiotics, drug interactions, order entry, verify medication orders
• Skills/Experience Preferred: Chemo experience, anticoagulants, Pharmacokinetic Dosing, Renal Dosing
• 1 Year Inpatient Pharmacy Experience Required.
• Travel Experience Preferred

Schedule:
Shift: 5x8's Rotating schedule, weekdays, some weekends (every 3rd weekend). If working overnights, 7 nights on, 7 nights off.
Shifts times:

• Day shifts: 6:30 AM-3:00 PM , 7:30 AM-4:00 PM, 9:00 AM-5:30 PM, 10:00 AM-6:30 PM;
• Evening shifts: 1:00 PM-9:30 PM, 2:30 PM- 11:00 PM
• Overnight shifts: 7:30 PM- 7:30 AM, 8:30 PM- 7:30 AM
