Work Environment

This role is primarily onsite, allowing you to interact directly with patients in their home environments. You will work regular business hours from 9 am to 5 pm, providing a structured schedule that facilitates work-life balance while ensuring optimal patient care. Key Responsibilities

• Conduct comprehensive psychiatric assessments and evaluations to accurately diagnose mental health conditions and develop individualized treatment plans.
• Provide ongoing medication management, including monitoring efficacy, adjusting prescriptions based on patient feedback, and ensuring safety through regular follow-ups.
• Collaborate closely with a multidisciplinary team, including primary care providers and psychologists, to create integrated care plans that address both physical and mental health needs.
• Offer crisis intervention and immediate support to patients experiencing acute emotional distress, ensuring their safety and providing necessary therapeutic interventions.
• Engage in community outreach initiatives aimed at raising mental health awareness, reducing stigma, and promoting our services to the broader Yonkers community.
• Deliver therapeutic support through individual counseling and psychoeducation to empower patients in managing their mental health effectively.
• Maintain meticulous and confidential patient records while adhering to HIPAA regulations to protect patient privacy.

Patient Population / Client Focus

Your patients will encompass a diverse demographic, including children, adolescents, adults, and families, each presenting unique mental health challenges. You will impact those coping with anxiety, depression, trauma, and mood disorders, providing care that is responsive to their specific cultural and social contexts. This role emphasizes understanding and addressing barriers to care faced by our patient population, thereby helping to achieve more equitable health outcomes.

Qualifications

• A Master’s degree in Nursing with a specialization as a Psychiatric Nurse Practitioner.
• Current state licensure as an Advanced Practice Registered Nurse (APRN) with psychiatric specialization.
• Strong communication and interpersonal skills, enabling effective interaction with patients, families, and other healthcare professionals.

Preferred Experience

• Previous experience in a community health center or a similar setting, showcasing familiarity with integrated behavioral health care models.
